Prior Visit forms & pretest instructions

Prior to visiting your cardiologist, it is best to be prepared in order to insure a quality visit in a timely manner. Therefore, it is important to have all the necessary paperwork filled out when you arrive for your appointment. Some other preparations you should make before arriving to your visit is always bring your current medications. Carry a list of your health care providers. If you have them, bring in copies of any recent lab results and any other test results from the past year, especially if the testing took place with a different health care provider. Write down a list of the questions you have about your condition and bring it with you to the appointment.
